New Student Forms

The following items must be submitted at the time of registration:

• New Student Enrollment Form  NEW STUDENT ENROLLMENT

• Oral Health Assessment  ORAL HEALTH ASSESSMENT

• Immunization records

• Birth Certificate/Passport (originals) - a copy will be made by the office

• Proof of Residency (utility bill - gas, electric - address verification may occur with home visits)
